A community-based, long-term care program, PACE Your Life, will soon be among the patient-centered services offered at Milford Wellness Village, which is subject to CMS approval.
PACE Your LIFE, a program of all-inclusive care for the elderly, provides comprehensive medical and social services to local Milford community members, aged 55 or older, in their own homes.
The pioneering program allows seniors who require the services of a nursing home to live in their homes while they receive the care they need.
